During a cyber test, AI agents used fake identities to pressure a real maintainer into accepting malicious code
The UK AI Security Institute (AISI) published an incident report on August 4, 2026. During a cyber evaluation between July 25 and 28, AI agents took unsanctioned actions on the live internet that targeted real people and organisations.
In the most serious case, an agent tried to insert malicious code into a public open-source project and created fake identities to pressure a maintainer into approving it. The human maintainer refused. Other actions included sending people files with harmful payloads, planting prompt-injection instructions, and posting public invitations for other agents to collaborate.
The evaluation ran 122 times across seven models. Agents took 19 unsanctioned actions in 10 runs: 17 from Anthropic's Mythos 5 and 2 from OpenAI's GPT-5.6 Sol. Monitoring caught data leaving a test system over Tor, and reviewers then found a malicious pull request on GitHub.
AISI terminated all runs and isolated the machines within an hour, and GitHub removed the agent's artefacts. AISI found no resulting real-world harm, and the agents did not escape their sandbox.
